EFFECTIVE 21ST APRIL 2022
All persons, except for those who can produce a valid medical proof of their unsuitability to receive COVID-19 vaccines due to health reasons, are required to be vaccinated according to the dosage schedule below in order to remain eligible for performing duties in the studio premises:
The COVID-19 vaccination requirements of the Vaccine Pass will be implemented in three stages:
1) The first stage will be implemented from 20th March to 29th April. Persons aged 14 or above are required to receive at least one dose of COVID-19 vaccine in order to use the Vaccine Pass to enter specified premises.
2) The second stage will be from 30th April to 30th May. Persons aged 18 or above are required to receive at least two doses of COVID-19 vaccine to continue to use the Vaccine Pass. The vaccination requirement for persons aged 14 to 17 remains the same as in the first stage.
3) The third stage will commence from 31st May. By then, if persons aged 18 or above have received their second dose of COVID-19 vaccine for less than nine months, their Vaccine Pass remains valid. However, if they have received their second dose for at least nine months, then they would need to receive a third dose to continue to use the Vaccine Pass. For persons aged 14 to 17, if they have received their first dose of COVID-19 vaccine for less than six months, they can still use the Vaccine Pass. Yet, if they have received their first dose for at least six months, they are required to receive a second dose to continue to use the Vaccine Pass.
A) LeaveHomeSafe App (“LeaveHomeSafe”)
B) Vaccination Record for the purpose of Vaccine Pass (best uploaded to the “LeaveHomeSafe” for smooth check-in) or COVID-19 Vaccination Medical Exemption Certificate
— QR code of local vaccination record using the “LeaveHomeSafe”, “iAM Smart” or “eHealth” mobile applications; or
— Printed local vaccination record with QR code;

To facilitate the use of the Vaccine Pass by recovered persons, they may download the recovery record QR code through the COVID-19 Electronic Vaccination and Testing Record System.
During the period, recovered persons may carry with them or present the recovery record QR code as a substitute for the vaccination record to enter specified premises under Vaccine Pass. Recovered persons may also save the recovery record QR code in the "LeaveHomeSafe" mobile application to facilitate scanning when entering a Vaccine Pass premises subject to active checking.
C) Mask up at all times in the studio and when taking the classes
* All clients must comply with the Hong Kong Government's rules and regulations upon entry to the studio. XYZ reserves the right to deny entry to anyone who fails to comply.
